I spent a couple hours over a period of time looking for the same thing for the Caldari and Gallente side of things, but to no avail. I figured no such map existed.
Doyce is obviously better at finding stuff on the interwebs than I am, because he found just such a map.
The map is by Yosarian, the same dude who put together the Minmatar/Amarr map. It was made in 2008, but should still be perfectly applicable to faction warfare in 2012 (I don't believe there have been any empire space system changes in that period of time.)
As I tweaked the Minnie/Amarr map, so too did Doyce, to label the mission hubs. Enjoy Doyce's find and his tweaks.
